La Vitamina D ed i suoi metaboliti svolgono la loro attività principale in quanto strettamente connessi all’omeostasi del calcio ed al metabolismo osseo. Essi però non limitano la loro azione a tale livello, ma esplicano anche rilevanti attività regolatorie su altri organi ed apparati. Vi sono evidenze sperimentali che dimostrano l’azione della Vitamina D sulle risposte immunitarie a livello dell’apparato respiratorio. Essa interviene infatti nella stimolazione e regolazione delle difese immunitarie antinfettive, sia innate che acquisite, modula la flogosi bronchiale e la sensibilità all’azione degli steroidi inalatori nell’asma, inibisce la produzione di metalloproteinasi e difende dallo sviluppo e da alcuni effetti sistemici della BPCO, inibisce i principali meccanismi pro-fibrosanti nel parenchima polmonare. A bassi livelli sierici di Vitamina D si associano forme più gravi delle malattie croniche polmonari ed in particolare un maggiore deterioramento della funzione respiratoria. Rimangono però molti aspetti da chiarire, in particolar modo riguardo alla utilità ed ai dosaggi della supplementazione con Vitamina D.

Summary
Vitamin D and its metabolites have a significant clinical role because of their interrelationship with calcium homeostasis and bone metabolism. But they did not limit their action at that level, as they also exert significant regulatory activity on other organs and systems. There are experimental evidences that show the action of vitamin D on immune responses in the respiratory system. It operates in fact in the stimulation and regulation of anti-infective immunity, both innate and acquired, modulates bronchial inflammation and sensitivity to the action of inhaled steroids in asthma, inhibits the production of metalloproteinases and defends from the development of certain systemic effects of COPD, inhibits the main mechanisms that induce fibrosis of lung parenchyma. Low serum levels of vitamin D are associated with more severe forms of chronic lung diseases and in particular with a greater deterioration of respiratory function. However many aspects remain to be clarified, especially regarding the utility and the dosage of supplementation with vitamin D.
